To
apply, please submit the following:
1.)
a copy of your current valid state
license(s)/certfication(s)
2.) a
copy of your current state
registration (if applicable in your
state)
3.) documentation authorizing you as
an independent practitioner (CSWs
-if applicable)
4.) verification of malpractice
insurance coverage
5.) complete the following
form
6.) submit $95.00 one time application
processing fee. Use the 'Buy Now'
Button to pay online by credit card or
check. Fax your credentials to
480-396-3213 or mail them to our
office address at the bottom of this
page.
Upon acceptance into the Network, you
receive a provider agreement which
establishes your relationship with
Find-a-Therapist, Inc. as an
Independent Contractor. You
will be billed an initial Virtual
Office Set Up Fee of $195.00 and an
annual maintenance fee of $125. There
are no other charges required to be

We provide
your Virtual Office which includes
Secure Chat capabilities, Online
Appointment Scheduling, Credit Card
processing and secure email accounts. You
receive a complete personal
orientation by phone and online to our
policies and procedures.
Support services are available if you
need assistance of any kind.
We manage advertising, promotional
campaigns, search engine submissions
and highlighting your online
services once you are accepted into
our provider network.
